Insurance Payers and Billing:
SMA Healthcare accepts Medicare, Florida Medicaid, and many major commercial health plans. It is your responsibility to update SMA Healthcare with the most current insurance information by providing a copy of your insurance card. As a courtesy, SMA Healthcare will bill your insurance plan for services rendered.
All co-pays, co-insurance and deductibles are your responsibility. When applicable, co-pays and co-insurance are expected at the time of service.
Please contact us for any questions or concerns that you may have regarding your account. All client responsibility questions regarding co-pays, co-insurance, and deductibles should be addressed with your insurance carrier.
Financial Assistance:
Financial assistance may be available to clients who have limited or no resources. SMA Healthcare utilizes a sliding fee scale that is based on income and family size. If a financial assessment reflects 200% or below of The Federal Poverty Guidelines for your household income/size, you may qualify.
To apply for financial assistance please print and complete Financial Assessment and bring to your next appointment. The Centers may request supporting documentation such as pay stubs, tax return, employment verification or other governmental agency verification as to your status.
