The Crisis Triage and Treatment Unit (CTTU) is a program designed to meet the crisis service needs of Flagler and Putnam Counties. Working closely with area law enforcement agencies, the CTTU serves individuals in crisis who are in need of support and/or transportation to and from a crisis stabilization stay. This program also serves self-referral clients for screening, triage and referral, court ordered and other community referred behavioral screenings for mental health and/or substance abuse.
Serves adults who have a mental health diagnosis. Therapy targets symptom management, improving coping skills, improving healthy relationships, and quality of life. Services start with assessment and individualized treatment planning and can be provided via individual and group therapy. Telehealth services are also available when appropriate. Family participation in treatment is encouraged to support the family in the recovery process. Outpatient services allow the individual to remain active with work, family and the community while they participate in treatment.
Adult Outpatient is for adults 18 years old and older who have substance use disorders.Clients can be voluntary or court ordered to participate. Court-ordered clients can participate in DUI court, drug court, Correctional Treatment Diversion Program (CTDP), or can be referred by probation or child welfare agencies. Services start with assessment and individualized treatment planning and can be provided via individual and group therapy. Telehealth services are also available when appropriate. Family participation in treatment is encouraged to support the family in the recovery process. Outpatient services allow the individual to remain active with work, family and the community while they participate in treatment.
Adolescent Outpatient Program is for youth ages 12-17 with substance use disorders. Services start with assessment and individualized treatment planning and can be provided via individual and group therapy. Telehealth services are also available when appropriate. Family participation in treatment is encouraged to support the family in the recovery process. Treatment is specifically directed toward repairing relationships, reducing impulsivity, developing social and life skills, and providing treatment interventions for the youth to remain substance-free. The goals of the program include establishing long-lasting recovery from drugs and alcohol and rebuilding a person’s skills to facilitate recovery and reconnection to the community.
The Medication Assisted Treatment (MAT) Program serves adults with opioid, stimulant, and alcohol use disorders in an outpatient setting. The program offers buprenorphine, naltrexone, or Vivitrol in combination with group and individual therapy, case management, and peer support.
Florida Assertive Community Treatment (FACT) program provides comprehensive support services to adults who have severe and persistent mental illness and have had multiple psychiatric hospitalizations. A multidisciplinary team consists of a psychiatrist, nurses, counselors, case managers, and peers, who deliver a wide scope of services and crisis assessment and intervention 24 hours a day, 365 days a year. With most services being provided in the community, individuals who might otherwise be relegated to life-long hospital stays are able to maintain independent living in the community of their choice.
Psychiatric Medication Outpatient Program (Med Management) provides medical support services to individuals with severe and persistent mental illness and co-occurring disorders. The goal is recovery and reconnection to the community. Services include psychiatric evaluation, treatment planning, medication management, and nursing services.
SMA Healthcare’s Drop-in Centers are available to anyone with mental health issues. Drop-in centers provide a safe and supportive place where those struggling with mental health issues can socialize with others, learn new skills, and participate in various activities. The Drop-in Center hours of operation are Monday-Friday from 8:30 a.m. to 4:30 p.m
Adult Targeted Case Management services are provided in an outpatient and home-based setting to assist the individual in gaining access to needed medical, social, educational and natural support, and other related services. The goal is to promote independent functioning through coordination of support services.
Treatment court programs are specialized court docket programs that allow individuals to enter long-term substance use treatment and agree to court supervision. Treatment court programs use a team approach to serve participants.
Family Treatment Coordinators (FTC) works with parents involved in the child welfare system who are experiencing substance use and/or mental health problems and are at risk of losing their children. Family Treatment Coordinators assess and link clients to treatment/services and provide case management services until the client is engaged in the services.
The Family Intensive Treatment Team (FITT) is a comprehensive in-home treatment and case management program that provides help to parents involved with child welfare (DCF/CPC) due to substance misuse and/or mental health problems. Peer Recovery Specialists are also used in the FITT Program to provide support and help families connect with the world of positive sober support in their local community.
The Youth and Family Tree program is a community-based program in Putnam County for adolescents and young adults ages 12-25 who are at risk or currently struggling with substance use and/or mental health.
Prevention serves adolescents and adults through education and development of substance use harm reduction skills. The goal of prevention is to ensure that community members are aware of growing trends and dangers of new and existing substances that are affecting individuals, families, and the environment. Prevention services are provided in various capacities to include schools, churches, community centers, and attending community events.
Outreach services are primarily education-based and designed to connect individuals with services they would benefit from. Outreach is provided in a variety of community and office-based settings.
Walk-in screenings are available to assist the community in determining what services a person needs. Through a brief assessment, SMA’s team will identify what services are needed for each individual client; the client will then be linked to those services.
Mental Health First Aid is an eight-hour training certification course which teaches participants a five-step action plan to assess a situation, select and implement interventions, and secure appropriate care for the individual. It introduces participants to risk factors and warning signs of mental health problems, builds understanding of their impact and overviews common treatments.

SMA Healthcare is one of the community partners that make Crisis Intervention Training possible, along with the National Alliance on Mental Illness of Flagler, Putnam, St. Johns and Volusia Counties and the Flagler, Putnam, St. Johns and Volusia Counties’ Sheriffs’ Departments. The program is a community effort designed to improve the skills of law enforcement to mental health emergencies with the goal of assuring safety, understanding, and service to the mentally ill and their families.
Prevention serves adolescents and adults in Volusia and Flagler counties through education and development of substance use harm reduction skills. The goal of prevention is to ensure that community members are aware of growing trends and dangers of new and existing substances that are affecting individuals, families, and the environment. Prevention services are provided in various capacities to include schools, churches, community centers, and attending community events. Prevention also offers Nurturing Parenting classes, that run over the course of 5 weeks via telehealth. These classes are DCF approved and can be offered to parents, guardians and caretakers, even if they are not involved with child welfare.
