Substance Use and Mental Health Outpatient Therapy for Adolescents and Adults

MH Therapy (OP):

Serves adults who have a mental health diagnosis. Therapy targets symptom management, improving coping skills, processing trauma, improving healthy relationships, and quality of life. Services start with assessment and individualized treatment planning and can be provided via individual and/or group therapy. Family participation in treatment is encouraged to support the family in the recovery process.

SU Therapy (OP):

Adult Outpatient is for adults 18 years old and older who have substance use disorders. Clients can be voluntary or court ordered to participate. Services start with assessment and individualized treatment planning and can be provided via individual and/or group therapy. Family participation in treatment is encouraged to support the family in the recovery process.

Adolescent Therapy (OP):

Mental Health Therapy serves children 5 and up, and substance use therapy serves adolescents 12 and up. The Adolescent Outpatient Program services include individual therapy with family involvement. Treatment is specifically directed toward repairing relationships, reducing impulsivity, processing trauma, developing social and life skills, and providing treatment interventions for the youth to remain substance free. Services start with assessment and individualized treatment planning and can be provided via individual and/or group therapy. Family participation in treatment is encouraged to support the family in the recovery process.

Medication Assisted Treatment (MAT)

The Medication Assisted Treatment (MAT) Program serves adults with opioid, stimulant, and alcohol use disorders in an outpatient setting. The program offers buprenorphine, naltrexone, or Vivitrol in combination with group and individual therapy, case management, and peer support.

Florida Assertive Community Treatment*

Florida Assertive Community Treatment (FACT) program provides comprehensive support services to adults who have severe and persistent mental illness. A multidisciplinary team consists of a psychiatrist, nurses, counselors, case managers, and peers, who deliver a wide scope of services and crisis assessment and intervention 24 hours a day, 365 days a year.

Psychiatric Medication Management

Psychiatric Medication Outpatient Program (Med Management) provides medical support services to individuals with severe and persistent mental illness and co-occurring disorders. The goal is recovery and reconnection to the community. Available for children 5 and up, and adults.

Drop-In Center

SMA Healthcare’s Drop-in Centers are available to anyone with mental health issues. Drop-in centers provide a safe and supportive place where those struggling with mental health issues can socialize with others, learn new skills, and participate in various activities. The Drop-in Center hours of operation are Monday-Friday from 8:30 a.m. to 4:30 p.m

Case Management

Targeted Case Management
Adult Targeted Case Management services are provided in an outpatient and home-based setting to assist the individual in gaining access to needed medical, social, educational and natural support, and other related services. The goal is to promote independent functioning through coordination of support services.

SOAR via Assisted Outpatient Treatment
SOAR is the method of assisting individuals who are homeless or in danger of becoming homeless apply for Social Security benefits. The SOAR processor acts as the appointed representative for the individual and can communicate with SSA without the individual being present. Most homeless individuals are denied benefits as they move around a lot and SSA can not find them. This prevents denials based on SSA not being able to contact the client.

Forensic
The Forensic Comprehensive Community Support Team provides services to persons with severe and persistent mental illness in and who have criminal charges. Case managers help navigate the system of care prior to incarceration and upon release from a prison sentence.

Treatment Courts

Treatment court programs are specialized court docket programs that allow individuals to enter long-term substance use treatment and agree to court supervision. Treatment court programs use a team approach to serve participants.

Child Welfare Services

Family Treatment Coordinators (FTC) works with parents involved in the child welfare system who are experiencing substance use and/or mental health problems and are at risk of losing their children.

The Family Intensive Treatment Team (FITT) is a comprehensive in-home treatment and case management program that provides help to parents involved with child welfare (DCF/CPC) due to substance misuse and/or mental health problems.

Youth and Family Tree

The Youth and Family Tree program is a community-based program in Putnam County for adolescents and young adults ages 12-25 who are at risk or currently struggling with substance use and/or mental health.

 

 

Prevention*

Prevention serves adolescents and adults through education and development of substance use harm reduction skills. The goal of prevention is to ensure that community members are aware of growing trends and dangers of new and existing substances that are affecting individuals, families, and the environment. Prevention services are provided in various capacities to include schools, churches, community centers, and attending community events.

Outreach Services

Outreach services are primarily education-based and designed to connect individuals with services they would benefit from. Outreach is provided in a variety of community and office-based settings.

Walk-In Substance Abuse/Mental Health Screenings

Walk-in screenings are available to assist the community in determining what services a person needs. Through a brief assessment, SMA’s team will identify what services are needed for each individual client; the client will then be linked to those services.

Primary Care

SMA Healthcare provides a comprehensive set of primary care services to individuals of all ages. This program provides access to care to improve the quality of community health through an integrated and holistic approach. Services include health screenings, point of care testing, medication management, immunizations, preventative care, and chronic disease education.
