Psychiatric Medication Management

Psychiatric Medication Outpatient Program (Med Management) provides medical support services to individuals with severe and persistent mental illness and co-occurring disorders. The goal is recovery and reconnection to the community. Available for children 5 and up, and adults.

Substance Use and Mental Health Outpatient Therapy for Adolescents and Adults

MH Therapy (OP):

Serves adults who have a mental health diagnosis. Therapy targets symptom management, improving coping skills, processing trauma, improving healthy relationships, and quality of life. Services start with assessment and individualized treatment planning and can be provided via individual and/or group therapy. Family participation in treatment is encouraged to support the family in the recovery process.

SU Therapy (OP):

Adult Outpatient is for adults 18 years old and older who have substance use disorders. Clients can be voluntary or court ordered to participate. Services start with assessment and individualized treatment planning and can be provided via individual and/or group therapy. Family participation in treatment is encouraged to support the family in the recovery process.

Adolescent Therapy (OP):

Mental Health Therapy serves children 5 and up, and substance use therapy serves adolescents 12 and up. The Adolescent Outpatient Program services include individual therapy with family involvement. Treatment is specifically directed toward repairing relationships, reducing impulsivity, processing trauma, developing social and life skills, and providing treatment interventions for the youth to remain substancefree. Services start with assessment and individualized treatment planning and can be provided via individual and/or group therapy. Family participation in treatment is encouraged to support the family in the recovery process.

Florida Assertive Community Treatment

Florida Assertive Community Treatment (FACT) program provides comprehensive support services to adults who have severe and persistent mental illness and have had multiple psychiatric hospitalizations. A multidisciplinary team consists of a psychiatrist, nurses, counselors, case managers, and peers, who deliver a wide scope of services and crisis assessment and intervention 24 hours a day, 365 days a year.

Case Management

Targeted Case Management
Adult Targeted Case Management services are provided in an outpatient and home-based setting to assist the individual in gaining access to needed medical, social, educational and natural support, and other related services.

SOAR via Assisted Outpatient Treatment (AOT)
SOAR is the method of assisting individuals who are homeless or in danger of becoming homeless apply for Social Security benefits. The SOAR processor acts as the appointed representative for the individual and can communicate with SSA without the individual being present. Most homeless individuals are denied benefits as they move around a lot and SSA can not find them. This prevents denials based on SSA not being able to contact the client.

Forensic
The Forensic Comprehensive Community Support Team provides services to persons with severe and persistent mental illness and who have criminal charges. Case managers help navigate the system of care prior to incarceration and upon release from a prison sentence.

Treatment Courts

Treatment court programs are specialized court docket programs that allow individuals to enter long-term substance use treatment and agree to court supervision. Treatment court programs use a team approach to serve participants.

Assisted Outpatient Treatment (AOT)

This program is for individuals serious mental illness who have a history of repeated discharge from inpatient care in stable condition, only to be readmitted under a Baker Act at least twice in a 36-month period. Participants will be referred to local psychiatric outpatient services to include medication management and counseling services in three counties.

 

Outreach Services

Outreach services are primarily education-based and designed to connect individuals with services they would benefit from. Outreach is provided in a variety of community and office-based settings.

Walk-In Substance Abuse / Mental Health Screenings

Walk-in screenings are available to assist the community in determining what services a person needs.Through a brief assessment, SMA’s team will identify what services are needed for each individual client; the client will then be linked to those services.
