Adult Adult-targeted Case Management services are provided in an outpatient and home-based setting to assist the individual in gaining access to needed medical, social, educational, and natural support, and other related services.
NAVIGATE is a comprehensive treatment program for people who have had a first episode of psychosis, a serious condition characterized by a disconnect from reality. The program provides services for up to two years and is for individuals residing in Volusia County who are between the ages of 15-36 years of age who have experienced their first episode of psychosis.
SOAR is the method of assisting individuals who are homeless or in danger of becoming homeless applyforSocial Security benefits. The SOAR processor acts as the appointed representative for the individual and can communicate with SSA without the individual being present. Most homeless individuals are denied benefits as they move around a lot and SSA can not find them. This prevents denials based on SSA not being able to contact the client.
The Forensic Comprehensive Community Support Team provides services to persons with severe and persistent mental illness in Volusia and Flagler Counties and who have criminal charges. Case managers help navigate the system of care before incarceration and upon release from a prison sentence.
SMAHealthcare’s Supportive Housing Case Management Program at My Place Apartments (MPA) is a partnership with the Volusia County Coalition for the Homeless. MPA provides a safe, sober, and supportive living environment for individuals experiencing chronic substance addiction who are homeless and who are living in unsuitable environments.
